The Gleason score uses a biopsy of the prostate to grade cell changes, indicating how aggressive the cancer is.29

Samples are analysed under the microscope and cells are categorised from normal to extremely abnormal/diffuse. Cells are graded with 1 and 2 being normal while 5 is the most changed with the highest risk of aggressive cancer.

A Gleason score is given by the two most commonly present types, and states which is the most common cell type first, eg a Gleason score of 9 (5+4). A Gleason score of 6 is low-risk grade 1 disease while a Gleason score of 8-10 is a grade 4-5 high risk cancer.
