Newham pharmacists develop patient 'wellbeing plans'

Accredited pharmacists in Newham are inviting patients with long-term conditions to develop their own 'wellbeing plans' under a scheme developed by their CCG.

The pharmacists can invite patients with two or more long-term conditions to take part in the Newham Self-Management Support Programme. These patients will have three one-to-one sessions with the pharmacist over the course of 12 weeks, where they will have the opportunity to talk about the issues that are most important to them – whether that’s increasing their personal understanding of their condition, working on their confidence in managing their medication, or improving their general health and wellbeing.

Personalised 'My Wellbeing Plans' might include making positive lifestyle changes by being more active, eating healthier or getting involved in their local community. Plans also focus on how these goals can be achieved, with pharmacists helping to connect people to local services and community groups that can provide ongoing support and encouragement.

Dr Ashwin Shah, a local GP and clinical lead for integration at NHS Newham CCG, said: “We want to help local people to take more control over their health and wellbeing. That’s why each ‘My Wellbeing Plan’ is completely driven by what each individual person feels is most important to them.

“We know that making lifestyle changes such as stopping smoking, eating healthy food and losing weight can be tough, but they can make a huge difference to your health and wellbeing. Your pharmacist is trained to help you to feel confident in your ability to manage your condition and to make sure that you have all the support you need to live your life to the full.”

The Newham Self-Management Support Programme has been developed by Newham CCG in partnership with  North-East London LPC and the London Borough of Newham, as well as local patient groups.
